Diagnostic Significance of Heparin-binding Protein (HBP) Antigen

Heparin-binding protein (HBP), also known as azurocidin or cationic antibacterial protein (CAP37), is a multifunctional protein with bactericidal function and chemotactic characteristics, which mainly exists in the secretory granules of neutrophils and azurophilic granules.

Diagnostic indicators for early-stage bacterial infections

Heparin-binding protein (HBP) is the first marker increasing in the process of bacterial infection. When infection occurs, bacteria invade blood vessels, stimulating neutrophils to release HBP, thus increases the content of HBP in plasma.

HBP alters the permeability of vascular endothelial cells through its strong positive charge, inducing plasma leakage and tissue edema (a characteristic manifestation of inflammatory response). In addition, HBP activates monocytes and macrophages to release inflammatory mediators such as tumor necrosis factor-α (TNF-α) and γ-interferon (INF-γ) and thus enhancing the body's immune response to infection or injury.
Compared with other conventional infection indicators such as PCT, CRP, IL-6 and leukocyte count, the variation of HBP appears earlier in infection with higher sensitivity, specificity, positive/negative detection rates and other comprehensive advantages. Dynamic monitoring of HBP concentration in plasma can be used to evaluate the level of sepsis and predict the risk of shock or cardiac arrest in severely infected patients. Long in short, HBP has great indicative significance to early diagnosis and treatment of clinical bacterial infections.
